Waivers

We have these waivers at the gym front desk for your convenience. If you would like to fill them out in advance, you can do so online or by printing out the form and bringing it with you.

Please fill out the two forms, General Liability Waiver and the COVID-19 Liability Waiver, for each customer who will be participating in activities at Chakstone Climbing Gym. Thank you!
